Certificates of Origin

Do you ship goods internationally? If so, you might find yourself in need of a Certificate of Origin. A Certificate of Origin is a document required by some foreign governments, which proves in which country the good was manufactured. In order to be acceptable, the Certificate of Origin must be signed by the exporter, notarized, and then validated by a locally designated agency, such as the Dayton Area Chamber of Commerce.

The chamber will only issue certificates for goods made in the United States during business hours (Monday – Friday, 8a – 4:30p EST).

How to receive your Certificate of Origin

  1. Download the Certificate of Origin form. (pdf). Please edit the form directly rather than printing the form and filling it out by hand. If you are unable to download the form, please call our office at 937-226-1444.
  2. Email the completed form to coo@dacc.org.
  3. Your signed and stamped Certificate(s) of Origin will be emailed to you within one (1) business day of submission. Rush jobs, required within four (4) hours of submission, will be charged an additional $10.

Cost and payment information

For a Dayton Area Chamber member in good standing, each Certificate of Origin is $10. Not a member? Learn more about becoming one!

For a non-Dayton Area Chamber member, each Certificate of Origin is $25.

Additional copies are $5 per page. Rush jobs, required within four (4) hours of submission, will be charged an additional $10.

The chamber will invoice your business for Certificates of Origin.

By submission of your Certificate of Origin to the DACC, your organization releases the DACC from any and all liability.  The DACC can only verify what the signee has presented as true and reliable.  If signee has falsified the origin of the goods made, DACC cannot be held responsible in any lawsuit.  No further vetting has occurred.
